Sentinel lymph nodes (marked in PURPLE) located in level I (arrow 1) are not always separately visual from the primary tumour with lymphoscintigraphy and SPECT-CT imaging caused by the location of that sentinel lymph node within the radioactive tracer hotspot of the tumour, known as shine-through phenomenon. Second lymph nodes in the drainage pattern might be wrongly suggested and harvested as SLN (arrow 2) or only SLNs in other neck levels (arrow 3) are harvested [33].
