Dose-finding in groups 3 and 5 *Missing data on blood pressure, in the dose-finding approach considered to be normal in the absence of evidence of hypotension; # Missing data on intubation score, effective sedation only judged by pre-intubation sedation level. In group 5, the 1.0 and 1.5 mg/kg dosing subgroups both contain 6 instead of 5 patients. This was due to initial uncertainty of the suitability of the data for the primary outcome in 1 patient in both subgroups. An extra patient in both groups was included to ensure a total of 5 patient with viable data on the primary outcome. After re-evaluation, the data of all 6 patients in both subgroups turned out to be suitable.
