4 Analyzing and discussing human factors affecting surgical patient safety I 123 Statistical analyses Descriptive statistics including mean (standard deviation (SD)) for continuous data and frequency analysis (%) for categorical data was performed to describe the frequency and rates of safety threats and resilience support events present in the MDR outcome reports. Analyses were performed with SPSS statistics 24.0 for Windows. For the secondary outcome, qualitative data analysis was used, by observing and counting which specific SEIPS subcategory codes (i.e. safety threat versus resilience support, and e.g. communication or teamwork) were discussed, per debriefing session. Narrative analysis was used to review the notes regarding the team’s general comments, feedback and take home messages made during the debriefings.
