In this way, it can also enhance the conversation between patient and loved ones. How do I use the LGP during my consultation? • Time is needed to discuss the LGP with patients and loved ones, usually at least 1 hour. If necessary, the topics can also be discussed in several sessions. • Start the discussion by asking what the patient considers the most important topic to be discussed. Then the other topics can be discussed point by point. • Patients often want to obtain a future view of their disease, their symptoms, their prognosis and their end of life. Therefore, it is advisable to discuss possible so-called “future scenarios” from each complaint and possibly record them in a so-called roadmap. • If themes or questions do not fall within your area of expertise, it may be useful to refer the patient and his relatives, for example, to a spiritual counselor or the palliative care team. 7
